Patients often have multiple CV risk factors. This program developed in collaboration with a Pfizer client team, a software developer, and subject matter experts, was designed to provide practices with a way to track the progress of patients toward their goals for hypertension, high cholesterol, and A1c levels. The program also reported progress of both individual patients as well as groups and related their goal status to the medications they received.
