This program – ID-AD – was intended to help case managers working with high-risk patients at health plans understand how Alzheimer’s Disease can complicate the management of other chronic conditions. The program was designed for delivery to health plans by account managers and consisted of 3 unbranded and 1 branded slide presentation. The unbranded presentations focused on recognition of AD and the value of therapy even as the disease continued to progress. A third deck, designed for use by the CNA in long-term care, discussed identification, and how therapy and delayed progression can ease management of symptoms.
