This is an educational program for embedded care managers designed to be delivered by Sanofi account managers. As healthcare delivery becomes more integrated, the role of the care manager takes on increasing importance, and many care managers who previously were located in the offices of health plans – and conducted their work telephonically – are now being located, or embedded, in primary care offices. These modules were developed to help these embedded care managers gain a deeper understanding of how the trend toward integrating care is one that helps them do what they are passionate about – provide better care for patients with diabetes.
